Wesley Village in Shelton to host panel of memory care experts for caregivers



SHELTON - Lifestyle Transitions Memory Care at Wesley Village is offering a free informational event for caregivers from 5-6:30 p.m. Thursday (April 7). 


The event will feature a panel of memory care experts from the Lifestyle Transitions team who will address the common topic families discuss when caring for a loved one with dementia or Alzheimer’s at home: How do I know it is time for my loved one to receive care outside of our home?


Caregivers interested in attending the event can RSVP to Rhonda Stasulli at 203-447-5454 or via email at rstasulli@umh.org

Lifestyle Transitions Memory Care team expands at Wesley Village in Shelton

SHELTON - Lifestyle Transitions Memory Care, a neighborhood of 68 apartments at Wesley Heights Assisted Living Community in Shelton, is proud to welcome a newly expanded team for the new year. 
In response to the addition of several new apartments now part of Lifestyle Transitions, the new team members will provide a greater depth of support and services for residents and their families.

New additions to the team include:
Patricia Kopf, RN (Supervisor of Assisted Living Services-SALSA)
Karen Wlcek, RN (Assistant Supervisor, SALSA Designee)
Jennifer Hemstreet, MSW, EMT (Social Worker/Resident Services Coordinator/Emergency Medical Technician)
Ann Panagulias, MM, ADCMC, CDP (Activities Director, Certified Dementia Practitioner)

Kopf brings to the team 32 years of nursing experience and a background in assisted living services and home care.

Wlcek, who is new to senior living, brings 35 years of experience in skilled nursing care and home care. Both clinicians will be joining the existing team of longtime, award-winning staff at Lifestyle Transitions: Sheila Franco, RN; Christina Fraga, RN; Ilisa Eskenazi, RN; and 25 certified nursing assistants.

Jennifer Hemstreet

Hemstreet, a social worker, previously worked with the Department of Children and Families and as a local EMT, providing emergency support services, training, and intervention. She is new to senior living and excited for the opportunity to serve residents and families at Wesley Village.

Ann Panagulias

Panagulias has worked in senior living for 12 years as an Activities Director and a Certified Dementia Practitioner. She brings a wealth of experience in engaging individuals with cognitive challenges and currently facilitates (in conjunction with the Alzheimer’s Association) an Alzheimer’s and Dementia Support Group for those caring for loved ones with dementia. Panagulias is also a professional musician and accomplished vocalist with an impressive operatic career around the world. She and her assistant, Hailey Monroe, provide meaningful, purposeful engagement for residents at Lifestyle Transitions and offer activities, events, and programs seven days a week, including every evening and weekends.

The team will work under the direction of Benjamin Schiano, Executive Director of Wesley Heights and Lori Pisani, Vice President of Assisted Living at Wesley Village.

“What makes our Lifestyle Transitions neighborhood such a special home for our residents is our caregivers," said Schiano. “The entire team is a fantastic resource to residents and their families, and the commitment to caring is truly exceptional. I am excited to see our team expand and look forward to working with everyone.”

The team will also be working on the creation of a series of virtual caregiver events which will be available to the general public throughout the winter and spring. Details will be forthcoming.

Rhonda Stasulli, Director of Campus Sales, is excited about the expanded support which will be available to prospective families looking for memory care options for their loved ones.

Wesley Village in Shelton breaks ground for expansion

SHELTON - Wesley Village has officially broken new ground.
Senior living campus officials today announced the expansion of Lifestyle Transitions, a specialized assisted living and memory support neighborhood, originally opened in January 2015.

Lifestyle Transitions offers a secure, supportive environment for those needing a higher level of attention and engagement, and for those in the early to mid-stages of memory loss. 
Unique to Wesley Village, Lifestyle Transitions provides a mission-based, relationship-centered approach to care, as well as: affordable pricing, a wander-protection system, 24-hour staffing with caregivers trained by the Alzheimer’s Association, nurses seven-days a week, on-site physician services, engaging activities, a variety of apartment styles for individuals or couples, education and support services for residents and families, and a specialized design to maximize independence. It also focuses on developing daily routines for successfully managing individualized physical, social, and cognitive challenges. 

The expansion will include 20 new specialized assisted living apartments in three added wings within the Wesley Heights community at Wesley Village. 
Additional spaces will include a family-style kitchen for baking; a great room for entertainment, exercise, art classes and more; lounges for reading, visiting and relaxing; and an enclosed outdoor courtyard for gardening, walking and fresh air. The design, colors, textures, lighting, and furniture were chosen with the help of senior living design consultants to create both vibrant and soothing spaces that best meet resident needs.

Tours will be available at Wesley Heights Independent Living Cottages and Lifestyle Transitions Memory Care.

Wesley Heights Cottages are Independent Living market rate cottage rentals. All cottages offer one-floor living.  Monthly market rental rates apply.

Wesley Village in Shelton celebrates Lifestyle Transitions' first anniversary

SHELTON - Residents, family members, and staff are celebrating the first anniversary of Wesley Village’s newest addition to their Shelton senior living campus: Lifestyle Transitions at Wesley Heights.  
Located in the newly renovated Northwest Wing of the Wesley Heights community, Lifestyle Transitions provides specialized assisted living and memory support options for aging adults. 
The community celebrated the first anniversary milestone with a family brunch, special activities, and other caregiver events throughout the month.
Lifestyle Transitions program director Lisa Kelleher reflected on her experience throughout the past year. 
“One of my favorite memories from our first year was celebrating the 100th birthday of our very first Lifestyle Transitions resident, Christina Sudol,” Kelleher said. “We threw a huge party and invited everyone to send birthday cards so we could present Christina with 100 cards. She was delighted.”
Sudol, who passed away this month, was well known and loved throughout the community. Her daughter, Patricia Day, expressed thanks for the care her mom received: “If the Lifestyle Transitions neighborhood had not been opened when it was, my mother would have had to go into a nursing home … it was there for her just in time.”
Unique to Wesley Village, the Lifestyle Transitions program is designed to support residents with cognitive impairment and increased social needs by engaging them in life and activities within the Lifestyle Transitions neighborhood as well as the larger Wesley Heights community.  
The program also focuses on developing daily routines for successfully managing individualized physical, social, and cognitive challenges.
“The Lifestyle Transitions program has greatly enhanced the quality of life of the residents in the entire Wesley Heights community,” said Faith Wajdowicz, Vice President of Clinical Services at Wesley Village. “By integrating residents from the Lifestyle Transitions neighborhood and the extended Wesley Heights buildings, it has taken the stigma of being on a ‘specialty unit’ off of these individuals, affording them specialized care that promotes independence as much as possible.”
Along with a mission-based, relationship-centered approach to care, the program offers: affordable pricing, a wander-protection system, 24-hour staffing with caregivers trained by the Alzheimer’s Association, on-site physician services, engaging activities, a variety of apartment styles for individuals or couples, and education and support services for residents and families. 
“The success of the program truly relies on the commitment of our great interdisciplinary team of staff, who is so supportive of the residents we serve,” Wajdowicz said.
This past summer, the Lifestyle Transitions neighborhood proudly implemented the Music & Memory program on the campus, making Wesley Village the first Music & Memory Certified Care Facility in the Valley, and one of only a few in the state. Also introduced during the past year was an Alzheimer’s & Dementia support group for caregivers, which the public is invited to attend every third Thursday of the month at 2 p.m.
“I am proud to be a part of an organization that has built a community dedicated to meeting the changing needs of residents, especially during difficult times of transition. To provide support for those with memory impairment and offer family members peace of mind is truly rewarding,” Kelleher said.

Wesley Village in Shelton to host Alzheimer's and dementia support group

SHELTON - Do you know or care for someone recently diagnosed with Alzheimer’s disease or a related dementia? Are you in need of support? You’re not alone! 

Caregivers are invited to Wesley Village from 2-3 p.m. Jan. 21 for an Alzheimer’s and Dementia Support Group. 
The group will meet on the third Thursday of every month and offer problem-solving techniques and helpful support for families and caregivers. 
It will be facilitated by Wesley Village staff trained by the Alzheimer’s Association. 
Your loved one is welcome to participate in activities in a secured environment while you attend the support group.

SHELTON - Lifestyle Transitions at Wesley Heights, an assisted living and memory support community on the Wesley Village Campus, will host a “Dinner with the Doc” event at 5:30 pm. Jan.14.
The discussion will be led by Wesley Village Medical Director Dr. Daniel Wollman, who provides both primary care and consulting services for memory and cognitive deficits. 
Wollman will facilitate a discussion for caregivers, offering strategies for dealing with memory loss and the aging process, and cognitive decline in older adults. 
A question and answer period will follow. 

This event will be held at Wesley Heights on the Wesley Village Campus at 580 Long Hill Ave.

Resident at Wesley Village in Shelton celebrates 100th birthday

SHELTON - Residents and staff at the Wesley Village senior living campus gathered Dec. 4 to celebrate Christina Sudol’s 100th birthday. 
Sudol, a resident in the Lifestyle Transitions neighborhood on the campus, was the first person to move in to the new wing, which was opened in January 2015. 
Staff from across the campus joined in the celebration and sent birthday wishes through 100 greeting cards, which were presented to Sudol by Lifestyle Transitions program director Lisa Kelleher. 
“We’ve been collecting cards from everyone – staff, residents, family and friends. Students from Park City Magnet School in Bridgeport even sent a bundle of handmade cards for Christina,” Kelleher said. “She was delighted!”

“I’m not 100,” Sudol argued, with a smile. 
“How about 50 x 2?” Kelleher suggested, to which Sudol agreed. 
Along with the birthday cards, the festivities included a beautiful birthday cake and Sudol’s favorite musical tunes: big band, classical, and Frank Sinatra. 
Wesley Heights Resident Services Coordinator Donna Brice also surprised Sudol, a dog lover, with a visit from her dog, Jonah, a Shih-Poo. 

Sudol received a proclamation from Mayor Mark Lauretti, recognizing her 100th year and sending her best wishes. Before cutting her cake, she stopped to thank her guests and enjoyed the afternoon, posing for birthday photos and reading her many birthday cards. 
“It was a big party,” she told her daughter.

SHELTON - Wesley Village is looking for volunteers to work alongside staff at “Lifestyle Transitions,” the new memory care neighborhood on their Shelton campus.

Located in the Northwest wing of their Wesley Heights assisted living community, “Lifestyle Transitions” offers specialized assisted living with memory support and focuses on developing daily routines for successfully managing individualized physical, social, and cognitive challenges.

Volunteer opportunities include: facilitating arts and crafts activities, assisting with and/or leading cooking and baking classes, “friendly visits” with residents, providing informal musical entertainment, gardening with residents, and more. If you are looking for a meaningful opportunity to connect with older adults and are interested in helping residents with memory loss enjoy a more full and vibrant life, please contact us today to learn more. We will match your talents and skills with the needs of our residents.

Project at Wesley Village in Shelton nears completion

'Lifestyle Transitions' memory support community being built
SHELTON - Phase two construction is nearly completed at Lifestyle Transitions, a specialized assisted living and memory support community on the Wesley Village campus. 

The Shelton senior living campus, which opened the memory care neighborhood in the northwest wing of their Wesley Heights Assisted Living Community in January of 2015, is adding an additional eight studio-suite apartments, a sunroom, and an enclosed outdoor courtyard. 
The project will be completed this summer, with apartments available this month.

Unique to Wesley Village, “Lifestyle Transitions” offers a mission-based, relationship-centered approach to care, as well as: affordable pricing, a wander-protection system, 24-hour staffing with caregivers trained by the Alzheimer’s Association, on-site physician services, engaging activities, a variety of apartment styles for individuals or couples, education and support services for residents and families, and a specialized design to maximize independence.

Wesley Village in Shelton to host open house for memory care program

SHELTON - Wesley Village will host an Open House from 4-6 p.m. July 15 at “Lifestyle Transitions,” a new memory care neighborhood located in the newly renovated Northwest Wing of its Wesley Heights community. 

“Lifestyle Transitions at Wesley Heights” offers specialized assisted living in a secure, supportive environment for those living with memory loss. 
The program is designed to support residents with cognitive impairment by engaging them in life and activities within the larger Wesley Heights community and providing encouragement, friendship, and a feeling of security. 
It also focuses on developing daily routines for successfully managing individualized physical, social, and cognitive challenges.

Unique to Wesley Village, “Lifestyle Transitions” offers a mission-based, relationship-centered approach to care, as well as: affordable pricing, a wander-management system, 24-hour staffing with caregivers trained by the Alzheimer’s Association, on-site physician services, engaging activities, a variety of apartment styles for individuals or couples, education and support services for residents and families, and a specialized design to maximize independence. 
Deposits are being accepted for all apartment styles, including new companion suite apartments, which will be available Aug. 1

Wesley Village in Shelton to debut Music & Memory program

SHELTON - Beginning this summer, individuals struggling from dementia and other cognitive and physical impairments will now have the opportunity to reconnect with family, friends, and caregivers through Music & Memory, a personalized digital music program to be offered on the Wesley Village Campus.

“Sincere thanks to the Valley Community Foundation for making this new program possible with the help of a $4,700 grant,” says Carlene Rhea, Director of Mission Development. 
“This grant enables our senior care campus to purchase iPods and other technology needed for music collection and storage and costs associated with staff training and instruction.” 

Rooted in extensive neuroscience research, the Music & Memory approach involves training care professionals to set up personalized music playlists, delivered on iPods and other digital devices, for those in their care. These musical favorites can then tap deep memories not lost to dementia, which enable patients and residents to converse, socialize, stay present, and feel like themselves again.

After completing three days of training with staff and volunteers, Wesley Village is the first Music & Memory Certified Care Facility in the Valley and one of only a few in the State of Connecticut. The Music & Memory program will be implemented first at their new Lifestyle Transitions memory care neighborhood on the campus and then in the Bishop Wicke Health and Rehabilitation Center.

Wesley Village in Shelton to host talk about memory care

'Dinner with the Doc' slated May 12
SHELTON - Lifestyle Transitions at Wesley Heights, a new memory support community on the Wesley Village Campus, will host a “Dinner with the Doc” memory care discussion at 5:30 p.m. May 12. 
The discussion will be led by Wesley Village Medical Director Dr. Daniel Wollman, who provides both primary care and consulting services for memory and cognitive deficits on the campus.

Wollman will facilitate a discussion for caregivers, offering strategies for dealing with memory loss and cognitive decline in older adults. 

This event will be held in the new Lifestyle Transitions memory care neighborhood at Wesley Heights, which is located on the Wesley Village Campus at 580 Long Hill Ave. 
Dinner will be provided and tours of “Lifestyle Transitions” will be available.
