Griffin Hospital in Derby to host Q&A about medications with pharmacists

DERBY - The Community Health Resource Center at Griffin Hospital invites the community to bring their medication questions to Griffin’s expert pharmacists March 4 from 11:30 a.m.-12:30 p.m. at the hospital.
Griffin Health Director of Pharmacy Services Lisa Jaser, Pharm D, and other members of the Griffin Pharmacy staff will lead a casual Q&A for any and all medication questions.
Medication errors injure hundreds of thousands of people every year in the United States. However, most medication errors can be prevented with open communication between the pharmacist and patient as well as the patient’s caregivers. 
Important information individuals should ask their pharmacist includes the name of the medication, what it is supposed to do, how to take or use the medication, what to do if a dose is missed, and what activities, foods, or other medications to avoid while taking the medication.

This program is part of a series of free, educational forums hosted by the Community Health Resource Center in an effort to help improve the health of the community.

Medicare is topic of presentation at Griffin Hospital in Derby

DERBY - In response to the great amount of interest in its first discussion on Medicare and related healthcare decisions, the Community Health Resource Center at Griffin Hospital will host another free presentation at 10 a.m. Oct. 29.

Joe Deegan, a counselor for Connecticut’s programs for Health insurance, Outreach, Information and Eligibility Screening (CHOICES), will host “Medicare CHOICES,” a presentation to help older adults and those eligible for Medicare and their caregivers make informed health insurance decisions and to help prevent, detect and report healthcare fraud and abuse. 

There will be information on Medicare, Medigap, Medicare Managed Care, Medicaid, Long Term Care Insurance and other related State and Federal programs.

This program is part of a series of free, educational forums hosted by the Community Health Resource Center in an effort to help improve the health of the community.

Griffin Hospital in Derby to host 'Here to Home' for patients, caregivers

DERBY - The Community Health Resource Center at Griffin Hospital will host a free presentation at 6 p.m. Sept. 26 about what patients and caregivers need to know when they go home from the hospital.

“Here to Home” is a comprehensive overview of information and the important safeguards patients and their loved ones need to consider when transitioning back home from a hospital stay. 
The presentation will help keep patients and caregivers safe and healthy as well as provide the confidence needed to navigate this relatively difficult time in the healing process.

The presentation will feature experts speaking about Case Management, Home Safety, Medical Alert Services, Medications, and Nutrition. 
The discussions will also highlight Griffin’s instructional videos about this transition, which can be found at griffinhealth.org/inpatient-care.

Griffin Hospital in Derby to host talk for caregivers of special needs students


DERBY - The Community Health Resource Center at Griffin Hospital will host a free talk to help caregivers of students with special needs advocate for their loved ones' education at 6:30 p.m. May 9.

Kevin Daly, Health Care Information Specialist with PATH Parent to Parent/Family Voices of Connecticut, will lead “The ‘Soft Skills’ of Special Education Advocacy,” a presentation for parents of students with disabilities who want to sharpen their advocacy skills. 
Topics include: best practices for school-home communications; how to get involved in the decision making process; how to prepare for school meetings (including PPT and 504); obtaining a free copy of your child’s school records; where to find help when you need it.

Daly has worked as a special education advocate with hundreds of Connecticut families for more than 20 years.
PATH/FVCT is a statewide grassroots parent organization that has been around for over 30 years. 
PATH/FVCT is dedicated to offering excellence in emotional and informational support to families, promote partnerships with families of diverse backgrounds and collaborate with healthcare providers and community organizations to improve services and policies that serve our children. 
PATH/FVCT has the support of two of the largest networks of families of children with unique abilities and special health care needs.
