DERBY - Griffin Hospital will offer a free Advance Care Planning Facilitators program next month.
The program will train individuals to help guide others in developing a plan for health care decisions. 
Advance Care Planning is a process of understanding, reflecting on, and discussing individual goals, beliefs, and values to determine one's health care wishes. This vital process assures individuals their health care choices will be honored in the event they are unable to speak for themselves. 

The program includes an online learning component and a class from 8 a.m.-4:30 p.m. Sept. 8 at the hospital, 130 Division St.
Registration for the program closes Aug. 28.
